Several states require a Federal Employment Identification Number (FEIN) when applying for unemployment benefits. We have provided the FEIN information below:

US Dept of Labor

Unfortunately,  we do not have any information about whether COBRA will be an option for our non-union employees. We know this makes it incredibly difficult, especially since Jack Cooper has always provided great benefits for employees and their families.  This is because we have to have an active plan in order to provide COBRA, and we do not know at this time how long the plan will remain active.  We will send additional information as available. In the meantime, we recommend looking into enrolling in other options if at all possible through the Healthcare Exchange, a new employer, Medicare if you are over 65, or your spouse's policy.  Many of these options are time sensitive.  We are working on getting letters from our plan to help show they are no longer eligible for benefits.  If you need assistance, please contact us at the hrcommunications@jackcooper.com email address.

Proof of Qualifying Life Event (QLE)

Most other healthcare options, like those listed below, require a Qualifying Life Event to enroll.  You will likely be asked for an Affordable Care Act compliant QLE letter showing you are no longer eligible for benefits.  This can be obtained through the Meritain portal login, found at https://account.meritain.com/ or on the TeamCare portal at http://www.myteamcare.org, also linked below. This may take several days before it is available.  Email HR if you are not able to find it hrcommunications@jackcooper.com

Benefit Coverage Options

Medicaid

If you qualify for Medicaid, you can apply for Medicaid at any time by one of the following options:

  • Online at HealthCare.gov 

  • Call 1-800-318-2596 (TTY 1-855-889-4325) to apply

  • In person at your local Division of Family and Children Services (DFCS) office, health department, or Social Security office 

Medicare

  • Medicare is for people who are 65 and over. To learn more, visit: https://www.usa.gov/medicare.

  • For Medicare, there are also time restrictions related to your date of job loss. Please ensure to look into this as soon as you are able.

The Health Insurance Marketplace provides a variety of affordable coverage options, and you may qualify for financial assistance based on your income. You typically have 60 days from the date of your coverage loss to enroll through a Special Enrollment Period (SEP).

Note that there are many websites out there that look like the Health Insurance Marketplace, but are actually private sites.  The official website is https://www.healthcare.gov/

Call TeamCare Customer Care at 1-800-323-5000 to request a Loss of Coverage letter if needed. If you're registered on TeamCare's website, you can also request the letter online. Click the button below to visit the website.
